2. Copy of Operating Authority
3. Copy of Certificate of Insurance– Ensure Flash Funding is listed as the Certificate Holder if funding is approved.
4. W9 Form
5. Copy of Driver’s License– For each Principal/Owner.
6. For Corporations or LLCs– Copy of Articles of Incorporation or Organization (or Charter/Registration Number).
7. For Sole Proprietors or Partnership – Copy of DBA or Partnership Agreement.
